How to Use Bazar
Find My Care
1. Click “Find my Care” and follow the Patient Path to identify your needs
3. Fill out the form & request an appointment.
2. Choose the Facility or Physician you’d like to see.
4. The Facility or Physician Practice will contact you to schedule your appointment.
Get Your Reward
After you’ve received care and your insurance has processed your claim(s), an Explanation of Benefits (EOB) will be sent to you either via mail or electronically…
1. After you have received your EOB, click “get my reward".
3. Bazar will process your submission. Check the status of your submission in “my submissions”
2. Fill out the required fields and take a picture or upload your EOB and click “submit”
4. Once verified and processed, Bazar will send your reward in the mail!
